It’s in the big swamp in eastern Swamp of Sorrows. The temple itself is just an island in the middle and then goes underwater.

Enter the temple and lower the stairs untill you find the room filled with water. Swim under water to reach the Broken Hall. Once in the Hall you can go to the surface (is a short swim so you dont need underwater breath buff). This Hall is safe and 3 tablets are in it.
