Why do holsters have a cant?
The cant of an ankle holster is a compromise between ease of draw and how concealed the firearm needs to be. A neutral cant provides a much smoother draw whether seated or bending over, while a forward cant helps the gun hug the shape of your leg and blend in more easily.

What’s the difference between Cant and OWB holsters?
A holster with a cant of that angle will have all the benefits that the high-ride OWB holsters that agents started wearing back in the day. If the degree of cant is a few degrees more than the fabled 15 degrees, it’s not going to make that much of a difference; what does matter is that it’s more than 10 degrees but less than 30 degrees.
