Killing Granny Rags counts as much as any other kill. You can still do that area by burning the chameo, stopping time (to make it easier), and either choke or sleep dart her.

IIRC you have to destroy the cameo, and then choke her out afterward/sleep dart her. Just ignore her from the beginning of the game and you wont even have to bother with her in the sewers.

Hello guys, some of you guys probably know this, but there is a way for you to fight Granny Rags while still having Ghost/Clean Hands. I saw this a few days ago, but didn’t try it myself until today. When entering the area, do not talk to either Granny Rags or Slackjaw.
How to save Slackjaw from Granny Rags ghost?
Save Slackjaw to get bonus money, grab the rune and other collectibles in the room, break the boards on the floor, and use the sewer to leave. NOTE:After the explosion, if you try to choke her before she stumbles, she will teleport a few feet away. You can try to choke her again, but it is risky that your time stop will end.
Is there a way to skip Granny Rags?
Skip Granny Rags completely, This way, you wont encounter them at all in the flooded district and you can continue your non-lethal run, If you’re doing that sort of thing (Source, I did it yesterday and got both Ghost and Clean Hands)
Is there a non lethal kill for Granny Rags?
From what I can tell the Granny Rags non-lethal hidden kill is like the natural rats counting as a kill, and may have been fixed in the same patch. And that was probably true in December of 2012. Unfortunately, the Dishonored Wiki doesn’t update their information (or fandom blog pages) with corrected information, such as the patched rats issue.
